Vanderbilt parent rating scale

For parents of children between roughly 6 and 12 years. Rate how often the behaviour applied during the past six months. Inattention and hyperactivity items are scored. Oppositional items are included for context and are optional here.

Inattention (9 items)

IN1Does not pay attention to details or makes careless mistakes

IN2Has difficulty sustaining attention to tasks or activities

IN3Does not seem to listen when spoken to directly

IN4Does not follow through on instructions and fails to finish tasks

IN5Has difficulty organising tasks and activities

IN6Avoids, dislikes, or is reluctant to engage in tasks that require sustained mental effort

IN7Loses things necessary for tasks or activities (school assignments, pencils, books)

IN8Is easily distracted by extraneous stimuli

IN9Is forgetful in daily activities

Hyperactivity / Impulsivity (9 items)

HY1Fidgets with hands or feet or squirms in seat

HY2Leaves seat in classroom or in other situations in which remaining seated is expected

HY3Runs about or climbs excessively in situations in which it is inappropriate

HY4Has difficulty playing or engaging in leisure activities quietly

HY5Is on the go or acts as if driven by a motor

HY6Talks excessively

HY7Blurts out answers before questions have been completed

HY8Has difficulty waiting their turn

HY9Interrupts or intrudes on others (butts into conversations or games)
